HRMC's social services department is available to assist you and your family with any concerns and can help find solutions to meet your needs. If preferred, a knowledgeable professional will meet with you prior to your leaving the hospital to ensure a smooth transition.

In collaboration with the physicians, nurses and other health care professionals, HRMC's social services staff helps access and coordinate many types of health services, such as:
Identification and assessment of needs
Liaison between you and the physicians and nurses
Discharge planning
Referrals for equipment and other community resources
Supportive and financial counseling
Advance directives
Crisis intervention
